Certification of an EENT Specialist, if loss of sight
Pictures showing the injured body member

In Case of Medical Reimbursement

Hospital’s Statement of Account with change slips
Original Official Receipts for medical, hospital, or doctor bills
Physician’s prescription/order for medicines or supplies if any
Medical Certificate
